His body is trying to hold on, but he wants to be with the tremulants. Knife me in, he wants to be sure he's set. Pencil me in is the opposite of knife me in. Knife is forever.

"Virulent hives, of bedpost piles"

These are the many machines next to his bed to keep him alive after he went so far to kill himself, virulent being parasitic, even though they are helping him he views it at taking him from the tremulants.

Forsaken deprived and wrought with fear talks about cerpins other forms were viewed in his life and coma life with the tremulants

Who turned it off, he is confused about his new life with the tremulants after his life(support) was turned off. He can't remember any of what brought him to the tremulants, because now he is actually dead.

Who turned it off, who brought me here goes back to earlier in the album, when talking of death it says

Cerpin did all this work in his mind to get to where he thought he should be by killing himself and now he is the last to know how he got to the messed up place he created for himself.
